Eligibility Criteria

People who have been diagnosed with a life-limiting illness may qualify for hospice services.

Signs a patient may qualify

A person who is experiencing these signs may be eligible for hospice care at home:

  • Two or more hospitalizations in the last year due to chronic illness
  • Diagnosed with a life-limiting illness
  • Multi-system failure or an increase in existing symptoms
  • Transitioning from aggressive treatment to comfort care
  • Weight loss of 10% or more in the last 6 months
  • Skin breakdown or wounds
  • Increasing weakness or fatigue
  • Two or more comorbidities present at the same time

Life-limiting diagnoses for hospice care

  • HIV/AIDS
  • Dementia
  • Cancer
  • Stroke/Cerebral Vascular Accident (CVA)
  • Congestive Heart Failure (Cardiac Disease)
  • Cardiopulmonary Disease (COPD)
  • Liver Disease
  • Neurological Conditions such as
  • Parkinson’s disease, Multiple Sclerosis, ALS, and Huntington’s disease
  • Renal Disease
