Heavenly Hands Hospice care team provides structure, convenience, and support to a patient’s life in ways that bring about calm and assurance. Our team consists of a medical director, specially trained nurses certified in hospice and palliative care, certified nursing assistants, medical social workers, chaplains, and volunteers. Your goals are our priority
Hospice is a philosophy of care that affirms life.  Our interdisciplinary team of medical professionals will design a clear and specific plan of care for you based on your goals. Essentially, everything that we will do for you and your loved one, whether that be medical care, assistance with grooming, bathing, feeding or spiritual and psychological support, will be clearly outlined. The plan of care describes the services to be offered along with the frequencies of each mentioned service.

If you or someone you care for are considering your care options, here are answers to frequently asked questions to assist you in your decision.

Q: Does someone have to be referred to hospice by a physician?

A: Anyone can refer a someone to hospice. Many people including family members, friends, and hospital case managers may refer someone to hospice. Learn about how to refer a patient to hospice services at home.

Q: Do I have to sign a Do-Not-Resuscitate?

A: A patient does not have to sign a Do-Not-Resuscitate to receive care from Heavenly Hands Hospice. This is a patient’s choice.

Q: What are your hours of operation?

A: Heavenly Hands Hospice offers service 24 hours a day/ 7 days per week. Our care team is available to address needs, concerns, and respond to referrals at any time.

Q: Can I receive care at home?

A: Heavenly Hands Hospice provides care for patients in their homes, assisted living or in nursing homes. However, if temporary inpatient care becomes necessary for symptom control or respite purposes, we have contracted with several local inpatient facilities to provide inpatient care at the determined time needed.

Q: What insurance do you accept?

A: You will not receive a bill for our services. Heavenly Hands Hospice accepts Medicare and Medicaid. Many private insurance companies provide coverage for hospice care, but as a precaution, patients should verify with their companies as to the scope of their individual coverage. Q: What are the hospice Levels Of Care?

A: We are proud to offer services across all four levels of hospice care.

  • Routine Homecare: Available wherever you call home (residence, nursing home and assisted living facilities, etc.)
  • Continuous Care: Medical management in the home for up to 24 hours a day during a crisis.
  • Short-Term Inpatient Care: When medical needs cannot be managed in the home.
  • Respite Care: Allows the patient a brief inpatient admission to provide caregivers “respite”.
